### Patch Changes

- SvelteKit 3 support: `useBreadcrumbs` and `usePageTitle` read `$app/state` (the removed `$app/stores` broke Kit 3 apps at import), and `caThemeHandle` derives its `Handle` type from `@sveltejs/kit/hooks` so it type-checks on Kit 2 and 3. The `@sveltejs/kit` peer range is now `^2.12.0 || ^3.0.0`. Hook return types are unchanged (`Readable`).
